Tractament de la leucèmia i el limfoma de Burkitt associats a la infecció pel VIH : resultats de dos protocols prospectius

Resum: Introducció i objectius. El tractament citostàtic intensiu ha millorat el pronòstic de la leucèmia i limfoma de Burkitt (BL), tanmateix es considera que la BL associada a infecció pel VIH té un mal pronòstic. El protocol PETHEMA-LAL3/97 va estar dissenyat per establir si la resposta al tractament i la supervivència dels adults amb BL eren comparables en pacients infectats i no infectats pel VIH. El protocol LAL3/LB04 PETHEMA va explorar si l'addició de rituximab a la quimioteràpia tenia un impacte en l'eficàcia i en la toxicitat en pacients infectats pel VIH. Diseny i mètodes. Els pacients van estar diagnosticats i tractats de forma uniforme i les dades recollides de forma prospectiva. La primera anàlisi del protocol PETHEMA-LAL3/97 va incloure cinquanta-tres pacients amb BL en estadi avançat. La segona anàlisi va extendre el temps de seguiment de pacients VIH-positius per aclarir el paper del tractament retroviral d'alta eficàcia (TR) en la supervivència d'aquests pacients. El protocol següent (LAL3/LB04) va avaluar trenta-sis pacients, estratificats en funció de la presència o no d'infecció pel VIH. En les tres anàlisis, es va documentar la resposta al tractament, la supervivència i la toxicitat en funció de la presència d'infecció pel VIH. Resultats. Primera anàlisi: No s'observaren diferències en la tasa de respostes completes (RC) entre pacients VIH-negatius (77%) o VIH-positius (71%). La supervivència global (SG) a dos anys va estar del 51% (intèrval de confiança -IC95%, 38%-64%) per als 53 pacients. La supervivència lliure de malaltia (SLM) a 2 anys per als 40 pacients que van obtenir la RC va estar del 60% (IC95%, 45%-75%). No es van trobar diferències en funció de la presència o no d'infecció pel VIH. Els pacients VIH positius que van rebre TR van mostrar tendència a tenir una SLM significativament millor (p=0. 051). Segona anàlisi: Es van incloure nou pacients (47%) que no havien rebut tractament o havien abandonat el tractament antiretroviral, 5 que ja el prenien prèviament al diagnòstic de BL i 4 que el van començar junt amb el tractament quimioteràpic. Tretze pacients (68%) van assolir la RC, la SG i la SLM per als pacients en RC a dos anys van estar del 46% (IC95%, 28%-64%) i del 71% (IC95%, 45%-94%) respectivament. La SG dels pacients en resposta al tractement antiretroviral (85% a 2 anys; IC95% 55%-98%) i dels no respondedors (27% a 2 anys; IC95% 3%-52%) van estar significativament millors per al primer grup (p=0. 035). Tercera anàlisi: Dinou dels pacients (53%) eren VIH-positius. Les seves característiques eren comparables a les dels pacients immunocompetents. Les tases de RC van estar del 88% i 84% respectivament per VIH-negatius i VIH-positius. El segon grup presentà una incidència superior de mucositis grau 3-4 (27% enfront 7% dels cicles, P = 0. 0005) i d'episodis infecciosos greus (26% enfront 8%, P=0. 0025). Tanmateix no s'observaren diferències estadísticament significatives en SG a 2 anys (82%, IC95% 65%-99% i 73%, IC95% 54%-92%, respectivament) ni SLM a 2 anys (93%, IC95% 82%-99% i 87%, IC95% 72%-99%, respectivament). Interpretació i conclusions. Els estudis confirmaren la factibilitat i efectivitat del tractament intensiu en pacients amb BL associada a infecció pel VIH amb resultats similars als de la població immunocompetent. Provaren que la resposta al tractament antiretroviral prolonga la SG en aquests pacients i mostraren la seguretat del tractament immunoquimioteràpic en pacients infectats amb eficàcia similar a la dels pacients immunocompetents.
Resum: Background and Objectives. Intensive multiagent chemotherapy has resulted in significant progress in Burkitt's lymphoma and leukemia (BL), however prognosis of HIV-associated BL is considered to be poor. PETHEMA-LAL3/97 protocol was designed to obtain evidence that response to therapy and survival of adult patients with BL were similar in HIV infected and in immunocompetent individuals. Furthermore, the LAL3/LB04 PETHEMA trial explored if the introduction of rituximab in the prior chemotherapeutic scheme had an impact in the outcome and toxicity of HIV-positive patients with BL. Design and Methods. Patients were uniformly diagnosed and treated and were included prospectively in the study database. The first analysis of the PETHEMA-LAL3/97 included fifty-three consecutive patients with advanced stage BL. The second analysis of the trial extended follow-up of HIV-positive patients in order to elucidate the role of HAART in their survival. The subsequent trial (LAL3/LB04) analyzed thirty-six patients, stratified according to HIV infection status. Response to therapy, survival and toxicity were evaluated according to the HIV infection status of the patients for the three analysis. Results. First analysis: There were no differences in complete remission (CR) rates between HIV-negative (77%) and HIV-positive patients (71%). The 2-year overall survival (OS) probability was 51% (95% confidence interval [CI], 38%-64%) for the 53 patients. The 2-year disease free survival (DFS) for the 40 patients achieving CR was 60% (95%CI, 45%-75%). No differences were observed according to HIV status. HIV-infected patients who received highly active antiretroviral therapy (HAART) seemed to have a slightly better disease-free survival than those who did not (p=0. 051). Second analysis: Nine patients who (47%) did not receive or discontinued HAART, 5 patients who had started HAART before the diagnosis of BL and 4 patients who started HAART along with chemotherapy were included. Thirteen patients (68%) achieved a CR, The 2-year OS probability was 46% (95%CI, 28%-64%) and the 2-year DFS for patients achieving CR was 71% (95%CI, 45%-94%). The OS of HAART responders (85% at 2 years; 95%CI, 55%-98%) and non-responders (27% at 2 years; 95%CI, 3%-52%) differed significantly (p=0. 035). Third analysis: Nineteen of the patients (53%) were HIV-infected. Their clinical characteristics were comparable to those of the HIV-negative patients. CR rates were 88% and 84% respectively for HIV-negative and -positive patients. HIV-infected patients presented higher incidences of grade 3-4 mucositis (27% vs 7% of cycles, P=0. 0005) and severe infectious episodes (26% vs 8%, P=0. 0025). However, there were no statistically significant differences in 2-year overall survival (82%, 95%CI 65%-99% and 73%, 95% CI, 54%-92%, respectively) or 2-year disease-free survival (93%, 95% CI, 82%-99% and 87%, 95% CI 72%-99%, respectively). Interpretation and conclusions. The studies confirmed the feasibility and effectiveness of intensive strategies in adult patients with BL with HIV infection with comparable results to those of the immunocompetent population. Proved that response to HAART prolongs OS in HIV-infected patients and showed that intensive immunochemotherapy can be administered safely to patients with HIV infection with remission and survival rates were comparable to those observed in HIV-negative patients.
