Models estadístics per al nombre d'implantacions i de naixements en les tècniques de reproducció humana assistida

Resum: Les tècniques de reproducció humana assistida (RHA) són una eina que en l'actualitat ajuden a moltes parelles amb problemes de fertilitat a tenir fills. Existeixen diferents tècniques a l'hora de realitzar la fecundació de l'oòcit i l'espermatozoide de la mateixa manera que existeixen diferents formes d'obtenir aquests gàmetes i de mantenir-los. Des del punt de vista estadístic és interessant poder mesurar l'eficàcia d'aquestes tècniques, és a dir, calcular les probabilitats d'experimentar un èxit. La majoria d'estudis relacionats amb aquest tema consideren un èxit en la reproducció humana assistida la implantació de l'embrió transferit a l'úter de la mare (el que sorgeix de la fecundació dels gàmetes) o el naixement d'un nadó viu. En l'estimació de les probabilitats d'èxit en les tècniques de RHA, el model Binomial no és un model adequat donat que no estima convenientment dites probabilitats. Aixó es deu a que l'escenari que planteja el model Binomial (esdeveniments i. i. d) no s'ajusta al que es dóna en el camp de la RHA, ja que, suposem que els diferents embrions que es transfereixen a l'úter de la mare amb alguna de les tècniques de reproducció humana assistida tenen la mateixa probabilitat d'implantar-se i desenvolupar-se fins al naixement d'un nadó viu, però s'ha estudiat amb detall que aquests no són independents i, per tant, l'èxit en un d'ells està relacionat amb l'èxit en un altre. Tot i això, el model Binomial és el model que normalment s'utilitza quan es volen donar a conèixer les probabilitats d'implantació dels embrions a l'úter de la mare o de naixement de nadó/ns vius (èxits) segons el nombre d'embrions transferits mitjançant alguna tècnica de reproducció humana assistida. Si es pensa en el cas més senzill, un model que suposi únicament que els diferents esdeveniments observats estan idènticament distribuïts, aquest model dependrà de tants paràmetres com variables es sumin (el model real del problema plantejat), però si no volem que el model proposat depengui de tants paràmetres hem d'imposar noves restriccions que s'ajustin convenientment a l'escenari que es presenta en el problema de la RHA. En aquesta línia, es proposen quatre nous models que surten d'imposar la condició de distribució idèntica entre els esdeveniments i, per cadascun d'ells, una condició pròpia pels diferents esdeveniments que fa que els models proporcionin diferents probabilitats d'èxit en les tècniques de RHA. Amb això, suposem que el model Binomial, que normalment s'usa, no és l'adequat per estimar les probabilitats d'èxit perquè, pel que suposa, no s'ajusta bé a l'escenari descrit pel nostre problema. Concretament, no és un bon model perquè s'observa que els diferents embrions que es transfereixen gràcies a les tècniques de RHA estan relacionats positivament, és a dir, que col·laboren conjuntament a l'hora d'implantar-se a l'úter de la mare un cop s'han transferit. També perquè vam observar que el nombre de gestacions gemel·lars éss superior al que s'esperaria sota el model Binomial. Amb aquest escrit volem estudiar els nous models que es proposen amb la finalitat de trobar si algun d'ells s'ajusta convenientment al problema que s'estudia, és a dir, si algun d'ells estima acuradament les probabilitats d'implantació d'embrions a l'úter de la mare i de naixement de nadons vius gràcies a les tècniques de reproducció humana assistida. Si això es dóna, amb l'escrit, es podrà proposar i argumentar que l'ús del nou model en l'estimació de les probabilitats d'èxit per les diferents tècniques de reproducció humana assistida és més adient que l'ús del model Binomial que, també gràcies a aquest, es demostrarà que no s'ajusta convenientment a l'escenari que planteja el problema que estudiarem. Després d'estudiar l'ajust de cada model proposat (incloent el model Binomial) sobre un conjunt de dades d'Espanya, Catalunya i altres països d'Europa estimant les probabilitats d'èxit sota el model real considerat i sobre cadascuna de les propostes i avaluant amb una prova estadística de bondat d'ajust com s'assemblen aquestes estimacions, es va concloure que el model Additiu era el millor model per estimar les probabilitats d'èxit que ens interessen. El model Additiu suposa, d'una banda, que els diferents embrions transferits estan idènticament distribuïts i, per tant, que tenen la mateixa probabilitat d'èxit i, d'una altra, que les interaccions de més de dues variables només depenen de les interaccions de les variables dos a dos de manera additiva. Amb aquesta idea, es proposa el model Additiu com a alternativa al model Binomial per estimar les probabilitats d'èxit en les tècniques de RHA i, amb aquest escrit, s'argumenta l'afirmació feta de que el model Additiu és més adient que el model Binomial i que els altres models proposats per estimar les probabilitats d'interès.
Resum: Assisted reproductive techniques (ARTs) are tools that currently help many infertile or sterile couples to have children. There are different techniques for getting the fertilisation of the oocyte and the sperm (embryo) in the same way that there are different methods to get these gametes and keep them in a good conditions. From a statistical point of view, we focus on the study of the effectiveness of these techniques by the estimation of success probabilities which, according to the information given by several studies surrounding this issue, a success in ARTs is defined as a pregnancy or a birth of a healthy baby after the embryos were transferred to mother uterus by a technique of assisted reproductive. In the estimation of the probabilities of success in ARTs, the Binomial model is not a suitable model because it does not give a proper estimations of these probabilities. However, the Binomial model is the most typically used when someone wants to show the probabilities of pregnancy and birth of a healthy baby (successes) according to the number of transferred embryos in the assisted reproductive techniques. If we think in the simplest case, a model which assume that the different observed events are only identically distributed, this model depends on as many parameters as variables we add (called the real model of the problem), but if we do not want that the model depends on many parameters, we must impose new restrictions which they fit properly to the problem explained for the ARTs. Following this trend, we suggest four new probabilities models that come to impose the condition of identical distribution between events and, for each of proposed models, a suitable and different condition defining the possible relation between events. In keeping with this, we assume that the Binomial model, which is the most frequently used model, is not a suitable probability model for estimate the success probabilities defined because it does not fit properly to the problem that we are describing. Specifically, it is not a good model because we suppose that the different embryos are positively correlated, that is, they collaborate together for getting a pregnancy or a birth of a healthy baby (for getting a success) after the transfer. Also, we observed that the number of twins pregnancies is bigger than the number obtained if the Binomial model would be the appropriate. Because of this reason, with this writing we want to study the new models proposed in order to find if any of them fitting properly the probabilities of interest, in other words, if any of them give estimations of probabilities of pregnancy or birth of a healthy baby according to the embryos transferred carefully. If this happens, we can argue the idea that other model, which was explained and applied to several real examples, is better than the Binomial for estimate the probabilities of interest. Furthermore, this writing can be used for proving this statement as well as for showing that the Binomial is not appropriate here. Finally, after studying the fit of each proposed model (including the Binomial) in a real dataset of Spain, Catalonia and other European countries by the estimation of the probabilities of interest with the real model (the model which only consider that the events are identically distributed) and with the proposed models and assessed the discrepancies between the estimations obtained, we concluded that Additive model is the best model for estimate the success probabilities in ARTs. The Additive model assumes that the different embryos transferred are identically distributed so they have the same success probability and, it also assumes that, the interactions of more than two variables depends on the interaction between pairs of variables additively. Owing to this conclusions, we propose the Additive model as an alternative of the Binomial model in the estimation of success probabilities surrounding the ART issues.
