Crecimiento y masa ósea en pacientes pediátricos trasplantados de hígado

Resumen: Introducción. El crecimiento después del trasplante hepático (TH) ha sido objeto de varios estudios. Las conclusiones no han sido consistentes. Poco se sabe del crecimiento a largo plazo de estos niños. La osteopenia es una complicación reconocida de la enfermedad hepática crónica pediátrica. Sin embargo no está claro si esta se debe a deficiencia de vitamina D, minerales, hormonas, o si es reflejo de la malnutrición de estos pacientes. El TH frecuentemente es la única oportunidad de recuperación, pero se sabe que causa reducción del contenido mineral óseo en pacientes adultos trasplantados. Métodos. Utilizamos un estudio retrospectivo para describir los patrones de crecimiento postrasplante en niños con TH, y para identificar los factores que afectan su crecimiento. Dichos factores fueron la edad, sexo, grado de retraso de crecimiento, tipo de trasplante, diagnóstico, función renal y hepática al primer año postrasplante, complicaciones y retrasplante, duración del tratamiento con esteroides, episodios de rechazo durante el primer año, nivel de colesterol y triglicéridos, y la inmunosupresión. Para el análisis estadístico, los datos se expresaron como escore de desviación estándar (DE). El z escore de DE de peso y talla fue calculado utilizando la siguiente fórmula: z escore = x - valor medio normal / DE de niño normal, donde x es el valor de un paciente y la DE la de la población normal de la misma edad cronológica. Utilizamos un estudio transversal para establecer la prevalencia de disminución de la densidad mineral ósea (DMO) en niños receptores de TH, e identificar los factores que afectan su DMO. Los factores estudiados fueron: estado nutricional, grado de retraso de crecimiento y su evolución, complicaciones y episodios de rechazo, duración del tratamiento con esteroides e inmunosupresión. La DMO de columna lumbar (L2-L4) se realizó utilizando absorciometría radiológica de doble energía (Lunar DPX-L) con alta definición, acorde a las instrucciones del fabricante. El Z escore de DMO para edad y sexo se calculó utilizando datos de niños españoles sanos. Entre Junio de 1985 y Diciembre de 2002, se realizaron 165 trasplantes en 143 niños (con 22 retrasplantes) en el Hospital Materno-Infantil Vall d'Hebron, Barcelona, España. Los criterios de inclusión para el estudio del crecimiento fueron: edad menor a 18 años al TH, ser solamente receptor de hígado, y tener mínimo un año de seguimiento en nuestra policlínica. De este grupo, 80 pacientes (43 mujeres, 37 hombres) cumplieron los criterios de inclusión. El promedio de seguimiento fue de 6 años, (rango: 1 a 10 años). Los pacientes se siguieron con intervalos de 4 a 6 meses. Obtuvimos las medidas antropométricas y las pruebas de funcionamiento del injerto en cada visita. Para el estudio de DMO, los mismos tres criterios de inclusión se utilizaron, además del consentimiento informado de los padres. Fueron 44 pacientes (21 hombres, 23 mujeres). La edad varió de 3 a 20 años (promedio: 11 años), y el tiempo promedio postrasplante fue 5. 3 años. Análisis Estadístico. Las pruebas de Kolmogorov-Smirnov y P-P Plot fueron utilizadas para confirmar la distribución normal de los datos. Posteriormente utilizamos pruebas paramétricas y no paramétricas. Los valores de probabilidad menores de 0,05 fueron considerados significativos. Todo el análisis estadístico se realizó con el SPSS, versión 10,0. Resultados y Conclusiones. 1. El crecimiento de acuerdo al z escore pretrasplante (< -2. 0 DE), enfermedad metabólica y el uso de esteroides por más de un año postrasplante, influyeron de forma negativa y significativa al segundo año postrasplante. 2. Encontramos DMO anormal en 27 de nuestros pacientes. Las causas fueron: estado de malnutrición, niveles bajos de 1-25 (OH)2 vitamina D, complicaciones y episodios de rechazo, y el uso de esteroides por más de un año.
Resumen: Introduction. Growth after pediatric liver transplantation (LT) has been the subject of reviews. The conclusions have not been consistent. Little is known on the long-term growth of these children. Osteopenia is a well-recognized complication of chronic liver disease in children. It is not known whether this bone disease results from a deficiency in vitamin D, minerals, or hormones, or whether it reflects the malnutrition that accompanies cholestatic diseases. Liver transplantation is often the only chance of recovery, but is itself known to cause a reduction in bone mineral content in adult patients. Methods. Using a retrospective study design we sought to describe post-LT growth patterns in liver transplant recipients and identify variables affecting their growth. Variables assessed were age, sex, pretransplant lack of growth, type of transplantation, primary diagnosis, liver and kidney function at one year post-LT, complications and retransplantation, prednisone therapy duration, allograft rejection episodes during the first year, cholesterol, triglycerides and immunosuppressive regimen. For statistical evaluation, data were expressed as standard deviation scores (SDS). Individual weight and height SD scores (z) were calculated according to the following equation: z score = x - mean value for normal / SD for normal children, where x is the value of an individual patient and SD is the standard deviation for the normal population at the same chronological age. Using a cross-sectional study design, we sought to estimate the prevalence of reduced bone mineral density (BMD) in pediatric liver transplant recipients and identify variables affecting their BMD. Variables assessed were: nutritional status, pretransplant lack of growth and its evolution, complications and allograft rejection episodes, prednisone therapy duration and immunosuppressive regimen. BMD of the lumbar spine (L2-4) was assessed by dual-energy X-ray absorptiometry (DXA) (Lunar DPX-L) with fine definition mode according to the manufacturer's instructions at the indicated time points. The age- and sex-matched Z scores of BMD were calculated based on the data of healthy Spanish children. Between June 1985 and December 2002, 165 liver transplants were performed in 143 pediatric patients (22 retransplanted) at the Hospital Materno-Infantil Vall d'Hebron, Barcelona, Spain. Inclusion criteria in growth study were: age under 18 at transplantation, liver-only organ recipients, and with follow-up longer than 1 year at the outpatient clinic. From this group, 80 transplant recipients (43 girls, 37 boys) were selected for growth study based on our inclusion criteria. Mean follow-up was 6 years (range: 1 to 10 years). Patients were followed up at 4 to 6 -month intervals on an outpatient basis. Anthropometric measurements were taken and graft function tests conducted at each visit. For BMD study, the same inclusion criteria were used, and informed consent was obtained from the families of all patients. There were forty four patients (21 boys, 23 girls). Age ranged from 3 to 20 years (median: 11 years), and mean duration since transplantation was 5. 3 years. Statistical Analysis. The Kolmogorov-Smirnov test and P-P Plot test were used to confirm normal distribution of data. When data were not normally distributed, nonparametric tests were used. Differences among groups were determined by the nonpaired t-test or the Kruskall-Wallis test. Probability values less than 0. 05 were considered to be significant. All statistics were performed with the SPSS program, version 10. 0. Results and Conclusions. 1. Growth according to baseline z score (< -2. 0 SD), metabolic indications for transplantation and steroid withdrawal after the first year post-LT showed the poorest growth with significant differences at 2 years post-LT. 2. We found abnormal BMD in 27% of our patients. There were multifactorial causes: malnutrition status, poor level of 1-25 (OH)2 vitamin D, complications and rejection episodes, use of steroids beyond 1 year post-LT.
