En este estudio, los receptores de un trasplante de sangre de cordón umbilical reciben un acondicionamiento similar, permite describir la cinética de la recuperación de las principales poblaciones linfocitarias post-trasplante de sangre de cordón umbilical y abordar sus factores predictivos. La presente Tesis Doctoral por tanto, describe los resultados obtenidos en cuanto a cinética y factores predictivos de la reconstitución inmune tras el trasplante de sangre de cordón umbilical en pacientes pediátricos y adultos con enfermedades hematológicas y no hematológicas. La reconstitución inmune es un evento que ha sido estudiado ampliamente tras el trasplante alogénico de progenitores hematopoyéticos. En la presente Tesis Doctoral me referiré a la reconstitución inmune como a la reconstitución de la población de linfocitos en números absolutos y de cada una de las subpoblaciones linfocitarias principales de manera cuantitativa. En los trabajos que conforman esta tesis se ha podido estudiar el valor predictivo de la administración de dosis de globulina antitimocítica y la cinética de la recuperación linfocitaria en estos casos. En resumen, los estudios que conforman esta tesis han demostrado el impacto que posee utilizar una determinada dosis de globulina antitimocítica observándose una RI más precoz, menor mortalidad y mayor supervivencia en pacientes que reciben una dosis menor de este fármaco en el régimen de acondicionamiento. |

In this study, the fact that most patients receive a similar conditioning allows us to describe the recovery kinetics of the major lymphocyte populations after umbilical cord blood transplantation and to address their predictive factors. The present Doctoral Thesis, therefore, describes the results obtained regarding kinetics and predictive factors of the immune reconstitution after the transplantation of umbilical cord blood in pediatric and adult patients with hematological and non-hematological diseases. Immune reconstitution is an event that has been extensively studied after allogeneic transplantation of hematopoietic progenitors. In this Doctoral Thesis I will refer to the IR as the reconstitution of the population of lymphocytes in absolute numbers and of each of the main lymphocyte subpopulations in a quantitative way. In the works that make up this thesis has been able to study the predictive value of the administration of doses of antithymocytic globulin and the kinetics of lymphocyte recovery in these cases. In summary, the studies that make up this thesis have demonstrated the impact of using a certain dose of antithymocyte globulin, observing an earlier immune reconstitution, lower mortality and greater survival in patients receiving a lower dose of this drug in the conditioning regimen. * This Doctoral Thesis contains the following sections: - Glossary of abbreviations -Discussion - Motivation of the thesis -Conclusions -Backgrounds and field of study -Bibliography -Introduction -Generalities umbilical cord blood transplantation -Justification of the study - Study hypotheses and objectives of the thesis. |
