Acute ischemic stroke is a major cause of morbidity and mortality in Europe, North America, and Asia. Its treatment has completely changed over the past decade with different interventional approaches, such as intravenous trials, intra-arterial trials, combined intravenous/intra-arterial trials, and newer devices to mechanically remove the clot from intracranial arteries. Large-artery intracranial atherosclerosis may be the most frequent cause of ischemic stroke worldwide. Traditional approaches have attempted to target the disease when it is already symptomatic. However, early detection of intracranial atherosclerosis may allow therapeutic intervention while the disease is still asymptomatic. [...]

Early arterial recanalization is a key point to achieve good functional outcome after ischemic stroke. Metabolic syndrome (MS) might lead to a worsening of stroke outcome in patients treated with tissue plasminogen activator (tPA) for several reasons: (a) first, MS, through its association with a prothrombotic state, could interfere with tPA-induced arterial recanalization and (b) secondly, many of the metabolic abnormalities related to MS could amplify cerebral ischemic damage. [...]
