Neurocognición, cognición social y funcionalidad en el trastorno bipolar

Resum: Tradicionalmente se había creído que los pacientes diagnosticados de un trastorno bipolar experimentaban una recuperación clínica completa entre los episodios agudos y no padecían una afectación en la funcionalidad general, a diferencia de los pacientes diagnosticados de esquizofrenia. Sin embargo, en la última década del siglo XX y sobretodo la primera del siglo XXI ha habido un aumento importante del número de estudios que han demostrado que esas suposiciones eran erróneas. Actualmente sabemos que los pacientes bipolares muestran déficit neurocognitivo y deterioro psicosocial aún en fases de estabilización clínica, además, estudios recientes detectan que estos pacientes presentan también alteraciones en la cognición social (CS). El presente proyecto tenía como objetivo estudiar la neurocognición, la cognición social y el funcionamiento social en los pacientes bipolares y en familiares de primer grado sanos, y para llevar a cabo este objetivo se diseñaron dos estudios. El primer estudio pretendía analizar si el déficit cognitivo de pacientes bipolares eutímicos estaba relacionado con la evolución clínica de estos. Las hipótesis de trabajo fueron que aquellos pacientes con mayor número de recaídas en general, y principalmente en fase maníaca obtendrían peor resultado en las valoraciones neurocognitivas. También se hipotetizó que aquellos pacientes con una historia de mayor recaídas con síntomas psicóticos presentarían peor rendimiento neurocognitivo. Para llevar a cabo este estudio se reclutaron 26 pacientes bipolares, en un periodo de eutimia de como mínimo 1 mes, del Centro de Salud Mental del Consorci Hospitalari de Vic. Se evaluó el rendimiento neurocognitivo de los 26 pacientes y se recogió la información clínica necesaria para el estudio de los historiales de los pacientes. Los resultados de este estudio mostraron que aquellos pacientes que tenían una historia de mayor número de fases de manía y un mayor número de ingresos previos en unidades de psiquiatría presentaban peor funcionamiento neurocognitivo, principalmente en el área de velocidad de procesamiento. El segundo estudio tenía como objetivos estudiar la cognición social en una muestra de pacientes bipolares eutímicos y en una muestra de familiares de primer grado. Así mismo, pretendía analizar el nivel de funcionamiento social de ambos grupos y observar las interrelaciones entre la cognición social y el funcionamiento social. Por último, nos propusimos comparar diferentes pruebas de evaluación de la CS con la finalidad de observar si aquellas más complejas y naturalísticas resultaban más útiles para el estudio de la CS en pacientes bipolares (presumiblemente con déficits más sutiles). Para realizar este estudio se reclutó una muestra de 31 pacientes bipolares eutímicos, 18 familiares de primer grado sanos y 31 controles sanos. Los pacientes asistían al Centro de Salud Mental del Consorci Hospitalari de Vic. Se evaluaron los tres grupos del estudio a con test neuropsicológicos, pruebas de CS y test de Funcionamiento Social. También se recogieron las variables de evolución clínica de los historiales de los pacientes bipolares. Los resultados de este estudio mostraron que los pacientes eutímicos con TB y los familiares de primer grado han presentaban déficits en ToM que iban más allá de los relacionados con un peor rendimiento neurocognitivo. En nuestra muestra de pacientes bipolares el tratamiento farmacológico, la duración de la enfermedad y un mayor número de recaídas depresivas se relacionaron con una peor ToM. Además, encontramos que los pacientes bipolares presentaban peor funcionamiento social que el grupo de familiares de primer grado y que el grupo de controles sanos, y que el déficit en el funcionamiento social de la muestra de pacientes se relacionó con la sintomatología depresiva en el momento del estudio y con un mayor número de periodos de eutimia con sintomatología subdepresiva.
Resum: Traditionally it had been believed that patients diagnosed with bipolar disorder experienced a complete clinical recovery between acute episodes and did not suffer from an impairment in the general functionality, unlike patients diagnosed with schizophrenia. However, in the last decade of the twentieth century and especially the first of the twenty-first century there has been a significant increase in the number of studies that have shown that these assumptions were wrong. We now know that bipolar patients show neurocognitive deficits and psychosocial impairment even in phases of clinical stabilization, but recent studies detect that these patients also have alterations in social cognition (SC). This project aimed to study the neurocognition, social cognition and social functioning in bipolar patients and healthy first-degree relatives, and to carry out this objective two studies were designed. The first study tried to analyze whether the cognitive impairment of euthymic bipolar patients was related to the clinical course of these. The working hypotheses were that patients with higher number of relapses in general, and especially in manic phase would get worse on neurocognitive outcome evaluations. We also hypothesized that patients with a history of major relapses presented with psychotic symptoms worse neurocognitive performance. To carry out this study 26 bipolar patients were recruited over a period of euthymia for at least one month, from the Center for Mental Health Hospital Consortium of Vic. Neurocognitive performance of the 26 patients were evaluated and clinical information was collected for the study of patients' records. The results of this study showed that patients who had a history of more phases of mania and a greater number of previous admissions to psychiatric units had worse neurocognitive functioning, particularly in the area of processing speed. The second study objective was to study social cognition in a sample of euthymic bipolar patients and a sample of first-degree relatives. It also tried to analyze the level of social functioning of both groups and observe the interplay between social cognition and social functioning. Finally, we proposed to compare different assessment tests from the SC in order to see whether the more complex and naturalistic are more useful for the study of SC in bipolar patients (presumably with more subtle deficits). For this study a sample of 31 euthymic bipolar patients attending the Centre for Mental Health Hospital Consortium of Vic, 18 healthy first-degree relatives and 31 healthy controls were recruited. The three study groups with neuropsychological tests, SC tests and Social Functioning test were evaluated. Clinical variables evolution histories of bipolar patients were also collected. The results of this study showed that euthymic patients with BD and first-degree relatives had deficits in ToM that went beyond those associated with poorer neurocognitive performance. In our sample of bipolar patients drug treatment, duration of illness and a greater number of depressive relapses were associated with worse ToM. Furthermore, we found that bipolar patients had worse social functioning than the group of first-degree relatives and the healthy control group, and that the deficit in the social functioning of the patient sample was associated with depressive symptoms at the time of study and with a larger number of periods of euthymia with subdepressive symptoms.
