Les dificultats neuropsicològiques són un símptoma habitual en els malalts afectats d'Encefalomielitis Miàlgica/Síndrome de Fatiga Crònica (EM/SFC) i de Fibromiàlgia (FM). Hi ha consens sobre l'alentiment de procés en l'EM/SFC però no és tant evident el compromís de la capacitat atencional ni de les funcions executives. La presència de dèficit cognitiu en la FM és més controvertida. No queda clara la contribució de l'alteració de l'estat d'ànim en la dificultat cognitiva. Objectiu: L'estudi pretén delimitar el perfil neuropsicològic d'ambdues malalties i la implicació del trastorn afectiu en el patró d'afectació. Material i mètode: Cent cinquanta-un pacients distribuïts en tres grups, 39 malalts amb EM/SFC, 74 malalts amb FM i 38 subjectes controls. S'administren criteris diagnòstics DSM-IV-R, test HAD i FIQ; i una valoració neuropsicològica on es valoren el domini atencional, velocitat en el processament de la informació, domini mnèsic i funcions executives. Resultats: Els malalts amb EM/SFC sense trastorn afectiu presenten alentiment en el processament de la informació. El Trastorn Depressiu Major (TDM) implica disfunció atencional, alentiment en la lectura de paraules i del color, alteració en la memòria d'evocació i d'aprenentatge, i en les funcions executives. Els malalts amb FM sense trastorn afectiu no presenten alteració cognitiva. El TDM en la FM s'associa a alentiment en la lectura de color, disfunció de la memòria immediata i a llarg plaç. Conclusions: L'EM/SFC es relaciona amb alentiment cognitiu mentre que la FM no presenta alteració de les funcions cognitives. La presència d'alteració de l'estat d'ànim condiciona el patró d'afectació cognitiva. |

The neuropsychological difficulties are common symptoms in patients suffering from Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS) and Fibromyalgia (FM). There is consensus on the slow processing speed in the ME/CFS but is not so obvious this compromise nor in the attentional capacity neither in the executive functions. The presence of cognitive deficit in FM is more controversial. It is not clear how the altered mood contributes in the cognitive difficulty. Objective. The aim of the study is to define the neuropsychological profile of both diseases and delimit the affective disorder involvement. Methods. One hundred and fifty-one patients were distributed in three groups: 39 patients with ME/CFS, 74 patients with FM and 38 control subjects. There were administered DSM-IV-R diagnostic criteria, HAD test, FIQ, and neuropsychological assessment, wich values the attentional domain, the speed in information processing, the memory domain and the executive functions. Results: Patients with ME/CFS without affective disorder show slow processing speed. The Major Depressive Disorder (MDD) involves attentional dysfunction, slowing in the reading of words and color, impaired recall memory and learning and executive functions. The patients with FM without affective disorder do not have cognitive alteration. The MDD in patients with FM is associated with slower color reading and an immediate and long term memory dysfunction. Conclusions. The ME/CFS is associated with slow processing speed while the FM has not altered cognitive functions. The presence of mood altered determines the profile of cognitive impairment. |
