Introduction: It has been postulated that NGAL is a trustworthy and early biomarker of Acute Renal Dysfunction (ARD). However, few studies have been carried out in the Intensive Care Units which characterized by 50% of ARD showing up during septicemia. As biochemical studies of NGAL reveal that its origin might not be just renal, but that is also related to inflammation or sepsis, the purpose of this study is to clarify the role of NGAL as renal biomarker in the critical patient. OBJECTIVES: 1) To determine whether NGAL level over 150 ng/mL is an early biomarker of ARD and a prognostic factor for the evolution of ARD in critical patients. 2) To determine whether NGAL can detect ARD in patients with creatinine below 0. 5 ng/mL. 3) To determine whether NGAL can distinguish between ARD due to sepsis from other etiologies. METHODS AND MATERIALS: A transversal study in 2 different days was assessed, and then patients were followed during their stay in the Intensive Care Unit up to a maximum of 30 days. We monitored daily the blood levels of serum creatinine. Urine samples were collected twice a week keeping them at -80ºC to measure the levels of NGAL (ARCHITECT, Abbott Diaagnosis), creatinine and urea later. Patients were classified according the Acute Kidney Injury Network (AKIN). RESULTS: Forty-six patients were included, 28% of them with ARD. The 72% of patients were classified AKIN-0, 11% AKIN-1, 11% AKIN-2 and 6% AKIN-3. We tested 529 urine samples. Patients were grouped according to a NGAL cut-off level of 150 ng/mL: - Seventeen patients (37%) had NGAL. |

Se ha postulado que el NGAL es un excelente biomarcador renal, precoz y fiable en la detección de la DRA. Pero, apenas existen estudios realizados en unidades de medicina intensiva en los que la DRA ocurre en un 50% durante un proceso séptico. Dado que los estudios bioquímicos del NGAL revelan que su naturaleza no es únicamente renal, sino que puede tener un papel en la inflamación e incluso en respuesta a infecciones bacterianas nos ha parecido oportuno intentar aclarar el rol del NGAL como biomarcador renal en el paciente crítico. OBJETIVOS: Determinar si un valor de NGAL. |
