Recientemente se ha aprobado el tratamiento endovascular (TEV) para el ictus por oclusión de gran vaso, lo cual ha cambiado radicalmente el pronóstico infausto de estos pacientes. Una de las contraindicaciones para el TEV es que el paciente llegue más alá de la ventana terapéutica (VT), que tradicionalmente era de 8 horas y actualmente es de 6 horas. Introducció: El gener del 2007 es va instal·lar un sistema de Telemedicina entre un hospital comarcal carent de neuròleg de guàrdia i el centre referència per a l'atenció dels pacients amb ictus agut, permetent l'avaluació especialitzada urgent a distància, l'administració supervisada de t-PA in situ i la selecció dels pacients candidats a trasllat urgent per a tractament endovascular. [...] Introduction: In January 2007, a telestroke system was established between a community hospital lacking a neurologist on call and a stroke centre. The telestroke system allowed urgent remote evaluation of the patient by a specialized neurologist, supervised thrombolytic treatment and a decision for urgent transfer to the stroke centre for endovascular treatment.
