Usefulness of molecular methods for detecting drug resistance and genotyping in mycobacterium tuberculosis

Abstract: La tuberculosis (TB) es una de las principales enfermedades infecciosas a nivel mundial, y la aparición y propagación de cepas de Mycobacterium tuberculosis, el agente causante, resistentes a distintos fármacos es una amenaza para la salud pública. Para mejorar el control de la TB es necesario el rápido diagnóstico de la enfermedad y la implementación de un tratamiento adecuado. Para determinar rápidamente el patrón de susceptibilidad, se han desarrollado distintos métodos moleculares que detectan mutaciones asociadas a resistencia. Asimismo, es esencial considerar el efecto de cada mutación en la resistencia fenotípica y en la evolución clínica. Por otra parte, el estudio de la transmisión de M. tuberculosis mediante genotipado es necesario para detectar relaciones epidemiológicas entre casos, y así controlar la propagación de la enfermedad. Por tanto, los objetivos de esta tesis son (1) evaluar la utilidad de nuevos métodos moleculares para detectar resistencias, (2) revisar los métodos moleculares utilizados para el manejo de la TB, y la relación entre la resistencia genotípica y fenotípica y la evolución clínica y (3) evaluar y desarrollar nuevos métodos para el genotipado de cepas de M. tuberculosis. En cuanto al primer objetivo, se evaluaron en cepas y muestras clínicas diferentes métodos moleculares basados en hibridación reversa en array y tiras, PCR multiplex y pirosecuenciación, comparando los resultados con los métodos fenotípicos de referencia. La sensibilidad de los métodos moleculares varió según el fármaco considerado, pero la especificidad fue generalmente alta. Los resultados de los diferentes métodos moleculares tuvieron una concordancia elevada, pero los métodos difirieron en cuanto a rendimiento, equipamiento y entrenamiento necesarios para la interpretación de los resultados. En cuanto al segundo objetivo, se revisaron los métodos moleculares utilizados para el manejo de la TB y la relación entre resistencia molecular y fenotípica y evolución clínica mediante una búsqueda extensiva de la literatura. Existen diferentes métodos moleculares comerciales para el diagnóstico, identificación, detección de resistencia a fármacos y estudios epidemiológicos, y cada metodología puede servir para más de un objetivo. La relación existente entre la resistencia molecular y fenotípica y la evolución clínica se ha estudiado para las mutaciones más comunes asociadas a resistencia a rifampicina, isoniazida, ofloxacino y kanamicina. Sin embargo, no se han realizado estudios sobre el efecto de otras mutaciones en la evolución clínica para éstos y otros fármacos. En cuanto al tercer objetivo, se evaluó un método basado en hibridación a micropartículas para el spoligotipado y detección molecular de resistencia a rifampicina e isoniacida. También se desarrolló un método de genotipado basado en pirosecuenciación para el análisis del polimorfismo de los lugares de inserción de la IS6110. La concordancia entre estos nuevos métodos y los de referencia -spoligotipado e IS6110-RFLP- en cuanto al agrupamiento o diferenciación de cepas fue casi completa. En conclusión, el diagnóstico y tratamiento de la TB ha mejorado significativamente en los últimos años gracias al desarrollo de métodos moleculares para detectar mutaciones que confieren resistencia, el estudio de la relación entre resistencia molecular y fenotípica y evolución clínica y el estudio de la transmisión de M. tuberculosis. Sin embargo, sería de gran utilidad disponer de nuevos métodos sensibles y específicos, fáciles de realizar y asequibles. Además, son necesarios estudios que incluyan un gran número de casos para identificar el conjunto de mutaciones que permitan predecir fallos en el tratamiento, y así poder diseñar un tratamiento adecuado y hacer uso racional de los limitados fármacos disponibles. Por tanto, aún son necesarios nuevos métodos diagnósticos, fármacos, y la completa elucidación de los mecanismos de resistencia en M. tuberculosis, entre otras estrategias, para eliminar la TB.
Abstract: Tuberculosis (TB) is one of the major infectious diseases worldwide, and the emergence and spread of drug resistant strains of Mycobacterium tuberculosis, the causative agent, is a public health threat. In order to improve TB control, rapid diagnosis of the disease and implementation of an adequate treatment are required. In order to determine the strain susceptibility pattern rapidly, several molecular methods have been developed to detect mutations associated with resistance. In addition, it is essential to consider the different impact that each mutation has on phenotypic resistance and on the clinical outcome. Furthermore, the study of M. tuberculosis transmission by genotyping is needed in order to detect epidemiological links among TB cases and outbreaks, and thus control the spread of the disease. Therefore, the objectives of this thesis are (1) to assess the usefulness of new molecular methods to detect drug resistance, (2) to review the molecular methods used for the management of TB, and the relationship between genotypic and phenotypic drug resistance and the clinical outcome, and (3) to evaluate and develop new methods for genotyping M. tuberculosis strains. For the first objective, different molecular methods based on reverse hybridization array and line probe assays, multiplex PCR, and pyrosequencing were evaluated on clinical strains and specimens, comparing the results with the standard phenotypic methods to detect drug resistance. The sensitivity of the molecular methods varied according to the drug, but the specificity was generally high. The results of the different molecular methods were highly concordant, but the methods differed in terms of throughput capacity, equipment and training required, and interpretation of the results. For the second objective, the molecular methods used for the management of TB and the relationship between molecular and phenotypic drug resistance and clinical outcomes were reviewed by an extensive search of the literature. Several molecular methods are commercially available for diagnosis, identification, detection of drug resistance, and epidemiology studies of TB, and each methodology can serve for more than one purpose. In addition, the relationship between molecular and phenotypic drug resistance and clinical outcomes has been studied for some of the most common mutations associated with resistance to rifampicin, isoniazid, ofloxacin, and kanamycin. However, there have not been studies of the effect of other mutations for these and other drugs on the clinical outcome. Finally, for the third objective, a method based on hybridization on microbeads aimed to perform simultaneously spoligotyping and detection of molecular resistance to rifampicin and isoniazid was evaluated. In addition, a genotyping method based on pyrosequencing aimed to analyse the polymorphism of the IS6110 insertion sites was developed. The concordance between these novel methods and the reference methods, spoligotyping and IS6110-RFLP, regarding the clustering or discrimination of strains was almost complete. In conclusion, the diagnosis and treatment of TB has significantly improved in the last years due to the development of molecular methods to detect resistance-conferring mutations, the study of the relationship between molecular and phenotypic drug resistance and clinical outcomes, and the study of transmission of M. tuberculosis. However, new highly sensitive and specific, easy to perform, and affordable methods could be useful for the management of TB. Furthermore, large studies are needed to identify the subset of mutations predictive of treatment failure, in order to tailor an adequate treatment and make rational use of the limited drugs available. Therefore, new diagnostics, drugs, and complete elucidation of the M. tuberculosis resistance mechanisms, among other TB control strategies, are still needed to achieve TB elimination.
