Background: Epicardial adipose tissue (EAT) is a visceral fat depot that surrounds the heart. It plays a key role in cardiac function, as it is the main source of non-esterified fatty acids (NEFA) and other lipids. However, EAT also acts as a transducer of inflammatory effects. An increase in EAT volume has been associated with a higher risk of heart failure or atherosclerosis, a situation commonly observed in patients with metabolic diseases such as obesity or diabetes. Despite this, the molecular mechanisms involved remain unclear. Materials and methods: This is a longitudinal (observational) study including 14 patients with severe morbid obesity who were scheduled to undergo bariatric surgery at Hospital de Sant Pau, and 14 healthy controls with normal weight. Anthropometric and biochemical parameters, as well as lipoprotein composition and function, were evaluated before and 12 months after the intervention. Results: Bariatric surgery normalized EAT volume and the levels of adipokines secreted by adipose tissue (leptin and adiponectin), but elevated levels of other inflammatory biomarkers persist. Bariatric surgery also improved several parameters of LDL function (proportion of LDL(-) and aggregated LDL) and HDL (by normalizing particle size). Conclusions: Bariatric surgery leads to a significant reduction in BMI and EAT volume, accompanied by an improvement in the inflammatory profile and lipoprotein functionality. |
