Al tratar la violencia política extrema en el siglo XX, muchos estudios historiográficos no han considerado tres aspectos fundamentales: primero, la diversidad de roles de quienes sufren esta violencia, más allá de la dicotomía entre la pasividad de la víctima incapaz y de la respuesta violenta directa; segundo, la relevancia de las viejas y pequeñas tecnologías, más allá de los nuevos y extensos sistemas tecnológicos de destruir y de proteger; finalmente, la tensión entre expertos y no-expertos en relación a estos sistemas. A correct approach to healthcare prioritisation requires accepting certain uncomfortable ethical and political truths. This article sets out and analyses some of these truths. Specifically, these are: that health prioritisation (i) is inevitable; (ii) means the denial of available resources to patients who need them for some kind of health benefit; (iii) is an issue of social justice and not of mere equitable access to the health system; (iv) is an ethical issue, which is not resolved through mere clinical and/or economic reasoning and will not be resolved through applying a morally painless ethical criterion; and (v) demands a commitment to solidarity above individual freedom.
